  The WebContainer can fail to add a web application to its
  internal mapping when there are a very large number of
  applications already started on the same virtual host. This
  error will result in the following exception in the log:
  java.lang.RuntimeException: FAILURE!!!!!!!!!!!!!!!!! couldn't
  add items

  The number of applications required for this problem to occur
  can vary. For example, the problem has been observed with
  around 600 applications on a single virtual host.
  

Problem conclusion

• The WebContainer has been modified to prevent this error from
  occurring when a large number of applications are installed.
  
  The fix for this APAR is currently targeted for inclusion in
