• Application requests fail due to application initialization
  having not happened when the first request to the application
  is from an include or forward from within another application.

  The first request to a resource within an application may fail
  if the application has not already been initialized.  Since we
  delay the application initialization, an include or forward
  from within another application could cause the problem.
  

Problem conclusion

• Code correction to initialize an application when the first
  request comes from an include or forward.
